Facility Use Application


Please review all Facility Use policies before submitting your application for building use. Written approval must be obtained, and all payments received prior to occupancy. You will be notified of approval via email.

*Off-hour use, or use of audio/visual, kitchen, musical, liturgical, or sound equipment may require additional fees, certifications, or the presence of St. Paul's staff or volunteers. 


I acknowledge that I have reviewed the Facility Use policies of St. Paul's Episcopal Church in Salem, Oregon. I hereby affirm that I am acting as an authorized representative of the above-named group, and agree that I will ensure that the group abides by the church's policies and requirements. I understand and acknolowedge that my group will assume the cost of repair for any damage caused. I acknowledge that liability insurance certificates must be provided to the church and that any deposits must be paide before our agreement can be considered valid. I understand that failure to provide insurance certificates or pay non-refundable deposits may result in the automatic cancellation of any approved building use.
